Appointment and New Patient Forms

To make an appointment with our urgent care clinic or specialists, contact our office at
(703) 658-7060 or use our online form below.

We will confirm your reservation request within the same or next business day. Please specify which clinic location (Annandale or Oakton) for which you are requesting an appointment. We can notify you by phone, email or text.

No appointment is required to be seen at the Annandale Urgent Care facility, but is welcome for your convenience. We are open 7 days a week with extended hours available and you can make an online appointment requests 24 hours a day!
